I, the parent or legal guardian of the athlete named above, give permission for my child to take part in Freeport AllStar Hounds activities, including practices, games, camps, events and travel.
I understand that flag football and cheerleading involve physical activity and a risk of injury. I accept these risks and release the Freeport AllStar Hounds, its coaches, volunteers and organizers from liability for any injury, illness or loss that may occur while my child participates, except in cases of gross negligence.
In an emergency, I authorize coaches to seek medical treatment for my child if I cannot be reached. I confirm the medical information I provided is accurate.
I give permission for photos of my child taken at team activities to be shared on team pages.
